The following information is needed to complete the webinar application. You must complete the form all at once, so we recommend putting this information in a Word document and then transferring it over to the form once you have all the information needed.

- Webinar description (200 words max)
- Learning outcomes (3 required). For example, “By the end of this webinar, participants will be able to ….” Learning Outcomes – Cheat Sheet
- Skill tags: Skill tags identify what skills recipients need to demonstrate or learn by the conclusion of the Learning Experience to receive their CEUs.
- Names, job titles, organizations, email addresses, phone numbers, biographies and photos of all facilitators.
- Including the student voice in the webinar is highly effective. Please consider inviting a community college student, who is 18 years of age or older, to be one of the facilitators.
- Five assessment questions with four possible answers, including one correct answer. All webinars must offer an exam or competency check. Assessments must be outcome-based and quantifiable because learners must pass an assessment to be eligible to receive CEUs.
